From Mentor, the drive to Painesville is about 15 minutes. There is no satellite office for marriage licenses in Mentor. All applications go through the Painesville courthouse. Marriage license applications must be submitted online first, and then you schedule an appointment by contacting Teresa Corbin in the Marriage Department.

Requirements for Mentor Residents
Both people must appear in person at the Painesville courthouse. Bring a valid photo ID. A driver's license, state ID, or passport works. You need to provide your Social Security number, though it is kept off the public record under ORC 3101.051.
The application asks for names, ages, addresses, birthplaces, occupations, parents' names (including mother's maiden name), and who will perform the ceremony. This is all required by ORC 3101.05. If either person was married before, you must provide a certified copy of the most recent divorce decree or death certificate if widowed.
Both people must be at least 18. There is no blood test and no waiting period. The license is good from the day it is issued and valid for 60 days anywhere in Ohio (for Ohio residents). Out-of-state couples must hold the ceremony in Lake County.
